Eligibility and clinical decisions

Every service requires an individualized assessment, clinical candidacy, informed consent, and applicable authorization. LUVION may decline, postpone, modify, or discontinue a service when clinically or operationally appropriate.

Telehealth

When care is delivered through telehealth, LUVION will obtain and document any consent required by Arizona law before the encounter. Telehealth is subject to clinical appropriateness, identity and location verification, privacy limitations, technology limitations, emergency instructions, and applicable prescribing rules. A website inquiry is not a telehealth encounter.

Pricing, estimates, and results

Published prices marked with a plus sign are starting prices. Final pricing may vary with product, dose, quantity, treatment area, and plan. Prices and programs may change. Individual results, duration, recovery, and complications vary; no outcome is guaranteed.

Booking, deposits, and cancellation

A $50 reservation deposit is collected for non-member and approved-model bookings and applied in full to treatment. Active Radiance Members in good standing enjoy deposit-free booking with dedicated Radiance Member Booking.

Please provide at least 48 hours’ notice when canceling or rescheduling to transfer the deposit or preserve it as account credit. Late cancellations and missed appointments forfeit the deposit. If LUVION cancels the appointment or the clinical assessment determines that treatment is not appropriate, the deposit will be promptly refunded.

Memberships

Radiance enrollment is governed by the signed membership agreement, including recurring billing, paid treatment balance, promotional credits, seasonal pauses, cancellation, non-stacking, and clinical terms. LUVION accommodates snowbirds and other seasonal residents through the seasonal-pause option described in the Radiance Membership Terms. Paid treatment balance does not expire. Promotional or loyalty credits may have a separately disclosed expiration date. Active-member pricing and benefits end upon cancellation and are suspended during an approved seasonal pause. If website language conflicts with the signed agreement, the signed agreement controls.

Clinical photography and marketing

Clinical photographs may be required for documentation and safety. Public marketing, portfolio, social-media, or educational use of identifiable images requires a separate written authorization unless another law permits the use. Model participation may require public-image authorization as an eligibility condition, but it does not replace treatment consent or medical-record documentation.

Communications

Email and SMS marketing are governed by the Email & SMS Terms. Public email and text channels are not appropriate for medical information or urgent concerns.

Intellectual property and acceptable use

LUVION names, logos, designs, photographs, copy, and other website content may not be copied, altered, republished, scraped, or commercially used without permission. Do not interfere with the site, attempt unauthorized access, or use it unlawfully.

Third-party services and limitation

Links to scheduling, payment, maps, or other third-party services are governed by their own terms and privacy practices. To the extent permitted by law, the website is provided without warranties and LUVION is not responsible for indirect or consequential loss arising from website use.

Arizona law and changes

These terms are governed by applicable Arizona and United States law. LUVION may update them by posting a revised effective date.

These website terms do not replace signed clinical, financial, privacy, photography, telehealth, or membership documents. Final documents should be reviewed by Arizona healthcare counsel and the medical director before clinical launch.
