Good Faith Estimate
& Financial Rights

Know the expected cost of your care.

LUVION is committed to transparent pricing. Federal law generally gives uninsured and self-pay patients the right to a written estimate of expected charges before scheduled healthcare services.

Your right to an estimate

If you do not have insurance or do not plan to submit a claim to insurance, you may ask for a written Good Faith Estimate of the expected charges for a scheduled service. LUVION will also provide an estimate when required after a qualifying service is scheduled.

Timing

When a service is scheduled at least three business days in advance, applicable federal timing requirements govern delivery of the estimate. You may request an estimate before scheduling, and LUVION will generally provide it within three business days of the request.

What the estimate includes

The estimate describes reasonably expected charges for the scheduled items or services known at the time it is prepared. It is not a guarantee. Your plan may change following examination, laboratory review, clinical findings, patient preference, or an unexpected need. You will be informed before elective additional services are provided whenever reasonably possible.

If the bill is substantially higher

If a final bill from a provider is at least $400 more than that provider’s Good Faith Estimate, you may be eligible for the federal patient-provider dispute-resolution process. Save a copy of your estimate and bill. More information is available at cms.gov/medical-bill-rights or by calling 1-800-985-3059.

Insurance status

LUVION may ask whether you have health coverage and whether you intend to submit a claim. Aesthetic and wellness services may not be covered by insurance. Published website prices are informational starting prices and do not replace an individualized written estimate.
