This notice describes how medical information about you may be used and disclosed and how you can access this information. Please review it carefully.
Your rights
- Request an electronic or paper copy of eligible medical and billing records
- Request correction of information you believe is incorrect or incomplete
- Request confidential communications or reasonable restrictions
- Request an accounting of certain disclosures
- Receive a paper copy of this notice
- Choose an authorized personal representative
- Complain without retaliation
Requests may be subject to verification, lawful limitations, and reasonable cost-based fees. When applicable, LUVION generally responds to access requests within 30 days and amendment requests within 60 days.
Your choices
You may tell us your preferences about sharing information with family or others involved in your care. Written authorization is generally required for uses not otherwise permitted by law, including most public marketing uses of identifiable treatment photographs. You may revoke an authorization prospectively in writing, subject to action already taken in reliance on it.
How information may be used and disclosed
As permitted or required by law, information may be used or disclosed to treat you, coordinate care, run clinical operations, obtain payment, support quality and safety, comply with public-health obligations, report suspected abuse or neglect, avert a serious threat, respond to workers’ compensation or lawful government requests, and respond to legal proceedings. Other uses or disclosures require authorization unless law provides otherwise.
Our responsibilities
LUVION will safeguard protected patient information, limit access to people and vendors with an appropriate need, provide required breach notifications, follow the privacy practices in effect, and make the current notice available upon request and on this website.
Arizona medical records
Medical records are maintained and released in accordance with applicable Arizona and federal law. Adult patient records are generally retained for at least six years after the last date of service; longer periods may apply to minors, particular records, contracts, or other legal requirements.
Secure communications
Ordinary email and text messaging may carry privacy risks. Do not use public website forms or ordinary email for medical records, treatment photographs, symptoms, urgent questions, or emergencies. Use LUVION’s secure clinical record system when available. Call 911 for an emergency.

You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights by writing to 200 Independence Avenue SW, Washington, DC 20201, calling 1-877-696-6775, or visiting hhs.gov/ocr/privacy/hipaa/complaints. LUVION will not retaliate against you for filing a complaint.
Changes to this notice
LUVION may change this notice and apply the revised notice to information already maintained as permitted by law. The current notice will be available in the office, through the secure clinical record system, upon request, and on this website.
This review copy must be reconciled with LUVION’s final HIPAA status, medical-director relationship, record ownership, business-associate agreements, and clinical workflows before the first patient encounter. The finalized notice and acknowledgment will be provided during intake.
